Important Information Regarding Your Sleep Study
If you have a sleep study scheduled, you will need to download, print and fill out the forms listed on the left. After completing these forms, please fax to 985-875-9480 or give it to your night technician at the time of the study.
If you need assistance completing any of the forms you may call the sleep solutions office at 985-875-7557 and select the "scheduling" option or seek assistance from the night technician.
Please complete the paperwork in advance to ensure a smooth sleep study experience.
* If you require a "sleep study partner" you will also need to complete the "Observation Study Partner" form.
A physician who is not part of Sleep Solutions will professionally interpret all sleep studies performed upon you (the Patient). And you, (the Patient) and/or your third party payer will be billed separately for this service by this interpreting physician.

Cancellation Policy
If you must cancel your appointment, please extend Sleep Solutions the courtesy of calling to inform us of the cancellation! Someone else may be waiting to benefit from this appointment and would love to fill this spot. Please call the scheduling department at 985-875-7557, NO LATER THAN 48 HOURS PRIOR to your scheduled appointment.
If you are a "No Show" or if you don't call to cancel your appointment as requested above, you are subject to a charge of $100.00. Thank you for helping us treat our patients as efficiently as possible.
